Job Details
We are seeking a Home Health RN Case Manager where you'll be managing a case load Home Health patients that are located in homes.
Territory is around the Delaware and Jay county areas.
Personal residence will be taken into consideration with territory.
Either homecare or hospice experience as a RN is required.
Location: Springfield, IL and Decatur, IL.
